Due to medical reason I need to break my lease and move?

My mother has terminal cancer, her condition has worsened. My dad has asked me to move back home so my wife and I can help take care of her and my 14 year old sister. My dad works 12 hr shifts and lot of over time to make ends meet. I have 2 months left on my apartment lease. But my family needs me to move in now.
